Application and Selection Process
Applications are currently closed.
Each individual will submit an application using the online form. We invite you to be creative with your application; one of the narrative questions provides the option to share a written response, video, audio, and/or poetry to showcase your commitment to this work. The application is broken into three sections:
- Demographic questions
- Eligibility questions to ensure you meet the program criteria.
- Narrative questions that focus on your truth, racial healing, and transformation work and invite you to talk more about how your work aligns with the framework.
Be sure to allow yourself plenty of time to gather all your materials and complete all sections of the application. We recommend you download the PDF version of the application from our website so that you can view all of the questions in advance and can then complete the application form in one sitting. Applications will be capped to the first 300 complete submissions we receive. Please do not submit more than one application. Because we receive so many applications, staff may not be able to assist all applicants in the final 24 hours before the submission deadline. Late submissions will not be accepted for any reason. If the deadline is extended for any reason, the extension will be posted on the program website.
After the application process has closed, representatives from the National Collaborative for Health Equity (NCHE); the national advisory committee (NAC); and other reviewers selected by the national program center will review each application based on the eligibility and selection criteria. If you are selected as a semifinalist, you’ll be required to participate in a virtual interview process. The virtual interviews are (TBD).
