“Advances in Evidence-Based Practices to Eliminate Disparities and Achieve Health Equity in Building Healthier Communities: Lessons Learned”

The 10th Anniversary Health Disparities Conference, titled Advances in Evidence-Based Practices to Eliminate Disparities and Achieve Health Equity in Building Healthier Communities: Lessons Learned, features lectures by individuals who are making significant contributions to improving healthcare access and to empowering vulnerable populations and marginalized communities.

This conference will allow participants to review and learn strategies focused on achieving health equity through access to care, prevention and treatment of major chronic diseases. Special session presentations will address interprofessional models of clinical, scientific, academic, and public health interventions that improve outcomes and eliminate health disparities and achieve health equity through treatment, advocacy, and policy development.
